Birth Certificates

Birth Certificates are available from the City Clerk's Office if:

- The birth occured in Boone County

- The birth occured in 2010 or later

Who can request a Birth Certificate:

  • The individual named, if 18 years of age or older.
  • A parent, if the parent's name appears on the Birth Certificate as mother or father.
  • A Legal Guardian, with the Court Order stating said guardianship.
  • The person's legal representative.
  • A department of the State or Federal Government
  • Any person upon order of a court

How can a person get a Birth Certificate?

  • In Person, at the City Clerk's Office, 401 Whitney Blvd., Belvidere, IL 61008. Monday - Friday,  8:00am to 5:00pm.  
  • By submitting a request by mail.   Certified copies will be mailed after request and payment have been received.

Any request for a certified copy of a vital record requires a copy of the applicant's current/vaild photo ID per the Illinois Division of Vital Records.  Please present a current/valid photo ID when applying in person, or attach a copy if applying via mail.

Cost of Certificates are as follows:

-$14.00 for the first copy

-$6.00 for each additional copy
