Welcome to the City of Belvidere Industrial Wastewater Survey.  

The City of Belvidere has a permit to operate its wastewater treatment plant by the Illinois Environmental Protection Agency.  In accordance with the requirements of that permit, we must conduct a survey of all of our industrial users every five years.  The information you provide by filling out this survey as accurate as possible will assist the City in complying with our IEPA permit.
